Please bring the following to your initial appointment:
A valid picture ID such as Drivers License
Minors MUST be accompanied by a parent/legal guardian
All Pediatric patients should bring a copy of their most recent immunization records
Insurance card, if applicable
All payment for procedures, co-pays, deductibles, and/or co-insurances will be due at the time of your visit
List of current medications with dosage and daily quantity or bring the bottle(s) with you

*These are the most common accepted healthcare plans in our office. However, there might be plans within a major company we are not in network with. If you do not see your plan listed or are not sure we take your plan, please check with your healthcare plan prior to your appointment to make sure your physician is in-network. Our office does not accept any Medicaid , Managed Care plans, Humana, or Coventry. Dr. Tatiana Pestana is not a Medicare participating physician.
Scheduling Tips for Dr. Tatiana’s and Dr. Elsa’s Patients:
Our patients are important to us!
We ask that our patients keep in mind that both Dr. Elsa and Dr. Tatiana maintain Part-Time office hours.
This means it may not always be possible to obtain same day appointments. Depending on the severity of an illness, patients may be seen within 24-48 hours. If we have a patient who feels that they are so sick that they cannot wait, we may need to refer them to the emergency room.
Also, because we try our best to be available for our existing patients when they are sick, we schedule a limited number of new patients per day.
This means that there may often be a several week waiting period for new patients to be seen. For this reason, we recommend that all NEW patients schedule their initial consultation when they are well. Once established with the doctor, obtaining a sick visit appointment will be much easier!
